Physician Office Referrals
**Please be aware our office is not wheelchair accessible. There are about 8 stairs that a patient must walk up to get to the main entrance. If your patient is unable to get up the stairs, we ask that you kindly refer them to a wheelchair accessible clinic. For the safety of our patients and staff, we cannot assist individuals up and down the stairs. Thanks for your understanding **
If you would like to book your patient to see one of our dermatology specialists, please fax a referral to 905-889-2006 with the following information:
Patients Name
Reason for referral
Patients cell number
Date of birth
OHIP card number
Patient’s address
Physician Name & OHIP number
Physician clinic address (if working at multiple locations, please ensure we have the correct clinic address)
Physician office and fax number
Please ensure you verify the patient’s information is correct to avoid delays.
Due to the number of referrals received, It can take 7-10 business days to process your referral. Once the referral is processed we will send you a fax with the date and time of your patient’s appointment.
To ensure your patient’s appointment is verified and our text confirmation is not deemed spam, please advise your patient we will be contacting them via text message through a generic number 2 weeks prior to their scheduled appointment. They will then receive a second reminder text message 1 week before their scheduled appointment. They must bring a current, non expired OHIP card when they come for their appointment.
Appointment Verification Via text Message
Please note the text message will come from a generic number, but will indicate it is from The Centre for Dermatology. Patients will only be required to enter their date of birth into the link provided to verify and see their appointment date and time. We will not ask for anything else.
The Centre for Dermatology WILL NOT ask for any other personal information such as Social Insurance number or Credit Card details.
** Please also advise your patient it can be up to an hour wait to see our specialist and they should plan accordingly **
